Kaiser Permanente seeks an LPN for our Virtual Care team supporting technology-enabled patient care. In this onsite role, you’ll provide remote patient monitoring, triage messages, and assist providers using digital care platforms. Responsibilities include conducting virtual check-ins, documenting in the EHR, coordinating follow-up care, and educating patients on technology tools. You’ll collaborate closely with RNs, physicians, and IT staff to resolve issues and improve workflows. Ideal candidates are tech-savvy, patient-focused, and comfortable working with clinical software in a fast-paced, innovative setting. Provide tech-enabled patient care via virtual platforms Perform remote patient monitoring and virtual check-ins Triage patient messages and route to appropriate providers Document care accurately in the electronic health record Educate patients on use of virtual care tools and portals Coordinate follow-up appointments and referrals Collaborate with RNs, providers, and IT on workflow issues Support troubleshooting of basic telehealth technology Follow clinical protocols and regulatory requirements Contribute to continuous improvement of virtual care processes Practical nursing (LPN) clinical skills Electronic Health Record (EHR) documentation Telehealth/virtual care platforms Remote patient monitoring tools Clinical triage and patient assessment Medical terminology and documentation standards HIPAA/privacy and security compliance Basic technical troubleshooting for patients Care coordination and follow-up management Written and verbal patient communication Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active LPN/LVN license in applicable state BLS certification Experience with electronic health records (EHR) Familiarity with telehealth or virtual care platforms
